Updated Waiver of Liability for existing clients
- I represent that my dog is in good health, is current on all required vaccinations (DAPP, Rabies and Bordetella), is current on Flea/Tick preventative treatment and has not been ill with any known contagious viruses in the last 30 days.
- I understand that vaccinations do not necessarily have 100% efficacy and that my vaccinated dog may contract a contagious virus/disease. I agree that should this occur I am responsible for my own pet’s care and medical attention.
- I agree to allow 7 days waiting period after my dog has had his/her vaccinations to allow the vaccine to reach full protection potential and to ensure that my dog has not had any negative reaction to the vaccines. Should I allow my dog to attend daycare in shorter than a 7-day wait period, I understand that my dog could be at risk of contracting a contagious virus/disease.

- I agree that by attending daycare I allow Ball and Biscuit to take photographs, videos or use images of my pet in print/ digital form for publication and/or promotion.
- I understand and agree that neither Ball and Biscuit nor any of its employees, staff or volunteers, will be liable for any illness, injury, death and/or escape of my dog provided that reasonable care and precautions are followed and I hereby release all of them of any liability of any kind whatsoever arising from or as a result of my dog attending daycare.
- I agree that my dog will be evaluated and deem suitable for daycare or not.
- I represent that my dog is sociable and has not harmed or shown threatening behaviours towards any person or any other dog. Should my dog display any unwanted behaviours, I understand that Ball and Biscuit reserves the right to remove my dog from the daycare and place my dog in a separate safe holding area.
- I understand that although all dogs are fully supervised, incidents of injuries may occur from playing with other dogs, which include but not limited to bites, scrapes, scratches and sprains.
- I understand that Ball and Biscuit reserves the right to permanently remove a dog from its premises and disallow future daycare attendance at any time.
- I understand that I am solely responsible financially or otherwise, for any harm or damage caused by my dog while my dog is attending any services provided by Ball and Biscuit.
- I allow Ball and Biscuit to contact my veterinarian or any other accessible vet clinic as deemed necessary should any injuries require medical attention. I agree that I am solely responsible for any medical expenses acquired for my dog.
- I agree to disclose any previous or current medical issues/ concerns of my dog so that Ball and Biscuit can make a determination of suitability for daycare.
- I release Ball and Biscuit from any liability should my dog injure another dog or person and accept medical and legal responsibility of my pet’s actions.
- I release Ball and Biscuit from any liability should death of my dog occur for any reason while in daycare.
- In the event that you, your emergency contact, or any person you have authorized to us in writing does not pick up your dog at closing time, late fees and /or boarding fees will be applied.
- Ball and Biscuit Dog Daycare reserves the right to disallow certain breeds to participate in the daycare.
